Neglect in Care: The Most Overlooked Safeguarding Issue

Apr 18, 2026

When we talk about safeguarding, the focus often falls on visible forms of abuse: physical, emotional, or financial. Yet one of the most damaging and frequently overlooked issues is neglect in care.

Neglect doesn’t always leave obvious marks. It can be subtle, gradual, and unintentional but its impact on vulnerable adults can be devastating. From missed medications to lack of emotional support, neglect quietly erodes dignity, health, and quality of life.

So why is neglect still under-recognized, and what can be done to address it?

What is Neglect in Care?

Neglect occurs when a caregiver fails to meet a person’s basic needs. This can happen in care homes, hospitals, or even within home care settings.

It may include:

  • Failure to provide adequate food or hydration
  • Poor hygiene support
  • Ignoring medical needs
  • Lack of supervision or safety measures
  • Emotional neglect and social isolation

Neglect can be intentional or unintentional, often caused by understaffing, lack of training, or burnout.

Why Neglect is Often Overlooked

Unlike other forms of abuse, neglect is not always easy to identify. Here’s why:

1. Gradual Onset

Neglect develops over time, making it harder to detect compared to sudden incidents.

2. Normalization

In busy care environments, poor practices can become normalized.

3. Lack of Training

Care staff may not recognize certain behaviors as neglect.

4. Underreporting

Victims may be unable or afraid to speak up.

Signs of Neglect in Vulnerable Adults

Recognizing the signs early is critical for safeguarding.

  • Physical Signs:

  • Malnutrition or dehydration
  • Poor personal hygiene
  • Untreated medical conditions
  • Bedsores or injuries

  • Emotional & Behavioral Signs:

  • Withdrawal or depression
  • Anxiety or fearfulness
  • Sudden behavioral changes

  • Environmental Signs:

  • Unsafe or unclean living conditions
  • Lack of basic necessities

The Impact of Neglect

Neglect can have severe consequences, including:

  • Deteriorating physical health
  • Mental health decline
  • Loss of dignity and independence
  • Increased risk of hospitalization

In extreme cases, neglect can be life-threatening.

Preventing Neglect Through Safeguarding

Effective safeguarding practices are key to preventing neglect.

1. Training & Awareness

Regular safeguarding training ensures staff understand their responsibilities and can identify risks early.

2. Person-Centered Care

Tailoring care to individual needs promotes dignity and respect.

3. Clear Reporting Systems

Encouraging staff to report concerns without fear is essential.

4. Regular Monitoring & Audits

Ongoing assessments help maintain high care standards.

5. Support for Care Staff

Reducing burnout improves quality of care and reduces risk of neglect.

Neglect may be less visible than other forms of abuse, but its impact is just as serious. Recognizing and addressing neglect requires vigilance, training, and a culture of accountability.

Safeguarding is not just about responding to harm, it’s about preventing it.
