Transforming Dementia Practice through Person-Led and Person-Centred Leadership

Drawing from Pat Hobson’s acclaimed book, Enabling People with Dementia: Understanding and Implementing Person-Centred Care, Fourth Edition, we help health and social care providers close the gap between theory and  practice in dementia care. 

Our Core Framework: The 3 Domains of the pathway

True person-led care requires looking beyond a clinical diagnosis to appreciating a person living with dementia's own reality. Pat Hobson developed the Unique Life, Unique World, and Unique Environment Model of Dementia Care, between 2009-2014 whilst delivering dementia training to over 100 care homes. This is a definitive, person-led pathway from diagnosis to end of life designed to unpack the complexities of dementia and maximise a person living with dementia's unique abilities through empowerment and enablement.

1. Unique Life

Dementia does not erase a person living with dementia's identity. This element of the pathway focuses on preserving personhood by deeply exploring a person's biography, values, personality, and life story. We move the workforce past clinical diagnosis to recognising and enhancing the unique person behind the diagnosis.

2. Unique World

To provide truly responsive care, we must enter the reality of the person living with dementia rather than forcing them into ours. This framework trains staff to recognise and understand behavioural indicators of distress as communication, preventing the "Malignant Social Psychology behaviours" (such as disempowerment or labeling) that compromise dignity, which are based on Kitwood's original theory  and explored in Pat Hobson's dementia book.

3. Unique Environment

A person’s surroundings can either disable or empower and enable them. We analyse how both the physical layout and the social psychology of an environment impact well-being. This pathway provides structural and interpersonal strategies to build spaces that stimulate meaningful activities, promote relaxation, and foster positive, dignified interactions as outlined in Pat Hobson's book.

  • Book-Aligned Curriculum: Modules are directly drawn from Enabling People with Dementia, unpacking complex concepts like Positive Person Work (PPW) and person-led and person centred communication.

  • Safeguarding Intersectionality: We train staff to identify behavioural indicators of distress early, translating frontline observations into preventative safeguarding action.  

  •  Real-World Application: Training utilises scenario-based, reflective learning that mirrors the high-pressure realities of day-to-day care.
