Everything You Need,
In One Place.
A dedicated one-stop platform for people living with dementia and their caregivers connecting you to the right support, services, and expert guidance at every stage of the journey. When you come to us, it means all else has failed and you feel that you have nowhere else to go.
You Should Only Ever Have to Come to One Place
People living with dementia and their caregivers often face challenges in navigating different systems and processes across a number of services and organisations which are fragmented. This can create additional pressures, isolation and stressful situations that result in delays in care delivery and support required to enable and empower people living with dementia to remain in their own home with person led support and services.
-
Based on PMH's Unique Life. Unique World and Unique Environment Model of Dementia Care Pathway that is explored in the Enabling People with Dementia: Understanding and Implementing Person-Centred Care by Pat Hobson. The pathway from diagnosis to end-of-life is a structured, person-led framework developed by Pat Hobson through years of experience working within dementia care and older people services. It holds organisations that support people living with dementia to account. The focus is more on how services fit with the person living with dementia and their caregivers rather than the other way round.

One Stop based on our Unique Life. Unique World and Unique Environment Model of Dementia pathway that holds every organisation to account
Person Led from Diagnosis to End of Life Ongoing Support
Our pathway ensures no one falls through the gaps from the moment of diagnosis through to ongoing, long-term care and end-of-life planning. The person leads their dementia pathway and remains in the 'driving seat' from diagnosis to end-of-life. Health, social care and allied professionals are facilitators of the care delivery and not the ones who lead on this, but the person and their loved ones.
Recognition & Diagnosis
Understanding the signs, getting a formal diagnosis, and knowing your right on what to expect in the first weeks, without having to wait for months without the right diagnosis and support.
Post-Diagnosis Support

Care Planning
